PLEASE NOTE: To obtain a certified copy of a birth certificate, the person making the request must be the individual, or the individual’s immediate family, i.e. spouse, father, mother, son, daughter, etc.
To obtain a certified copy of a death certificate with cause of death, the person making the request must be a member of the individual’s immediate family, i.e. spouse, father, mother, son, daughter, etc.
FORM MUST BE SUBMITTED WITH AN ENLARGED PHOTO I.D, AND MAILED TO ADDRESS ABOVE WITH CHECK FOR APPROPRIATE AMOUNT, AND INCLUDE A SELF-ADDRESSED AND STAMPED ENVELOPE. IF REQUESTING IN PERSON, THE OFFICE ACCEPTS CASH OR CHECKS ONLY. WE DO NOT ACCEPT DEBIT OR CREDIT CARDS.
